Can aggressive postoperative non-narcotic therapy replace narcotics in patients undergoing laparoscopic hysterectomies?

چکیده
Background Pain, although invariably present after surgical procedures, is not always well controlled. Medications from different analgesic groups are often used to control post-operative pain. Recently, attention has turned towards the optimization of non-opioid analgesics including NSAIDS and acetaminophen. After gynaecologic laparoscopy up to 80% of patients may require opioid analgesia. However, opioids can have adverse effects including nausea, vomiting, sedation, and respiratory depression. Thus the prudent surgeon attempts to use a narcotic-sparing approach to postoperative analgesia. Theoretically, aggressive non-narcotic analgesic administration will result in less narcotic use. Fortunately, both NSAIDs and acetaminophen are very effective in the control of moderate to severe pain and have few side effects. Our research question then is: “What is the post-operative narcotic use amongst women undergoing laparoscopic hysterectomy who receive aggressive non-narcotic therapy?”

Purpose. To compare narcotic versus nonnarcotic outpatient oral pain management after pediatric laparoscopic appendectomy. Methods. In a prospective study from July 1, 2010, to March 30, 2011, children undergoing laparoscopic appendectomy on a rapid discharge protocol were treated with either nonnarcotic or narcotic postoperative oral analgesia. Two surgeons in a four-person faculty group emplo...
متن کاملMultimodal Pain Management Protocol Versus Patient Controlled Narcotic Analgesia for Postoperative Pain Control after Shoulder Arthroplasty
Background: Our institution’s traditional pain management strategy after shoulder arthroplasty has involved the utilization of postoperative patient-controlled narcotic analgesia. More recently, we have implemented a protocol (TLC) that utilizes a multimodal approach. The purpose of this study was to determine whether this change has improved pain control and decreased narcotic utilization. ...

OBJECTIVE Patients undergoing craniotomies have traditionally received opiates for the management of their postoperative pain. The use of narcotic pain medications can be costly, can decrease early walking, can lengthen hospital stay, and can alter a patient's neurological examination results. The use of alternative pain medications such as cyclooxygenase-2 (COX-2) inhibitors may benefit patien...

HYPOTHESIS Patients undergoing warmed, humidified carbon dioxide (CO2) insufflation for laparoscopic cholecystectomy will (1) maintain a warmer intraoperative core temperature, (2) have their surgeon experience less fogging of the camera lens, and (3) have less postoperative pain than patients undergoing laparoscopic cholecystectomy with standard CO2 insufflation. DESIGN A double-blind, prosp...
